Join DFC for a forum on social determinants of health, screening for unmet needs, and community interventions to address health disparities. Presentations will begin with a broad overview of the social determinants of health and the correlation between these factors and health outcomes, as well as screening methods and how they can be incorporated into various practices. Next, we will take a deeper dive into how some of these social factors manifest themselves, and work that is being done in the community to address these issues.
Topics of discussion will include clinical screening, implementation of screening practices, adverse childhood experiences such as intimate partner violence and food insecurity, as well as community initiatives framed around this research.
